HOW TO FIND US
The surgery is situated at 93 Chapel Street which runs parallel with the
High Street in Billericay. Car parking for patients whilst attending the
surgery is at the rear of the surgery – however, space is extremely
limited and parking is at owner's risk.
Disabled
Access
We have a ramp at the front of the surgery for wheelchair access, prams
and pushchairs.
Registering with the Doctor
If you wish to join the Practice you will need to come into the surgery
in person with your passport and complete a registration form.
Alternatively, if you have a medical card, please bring it with you and
have full details of all members of the family who wish to register.
Each person will be asked to complete a new patient questionnaire and a
copy of your passport will be taken. In addition a New Patient Contract
will be issued to you for your completion and signature.
